By submitting this form, I acknowledge that dance and/or fitness classes involve certain risks and potential dangers and I voluntarily agree to participate in class and assume all risks associated with my participation. I hereby waive any and all claims and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in activities offered by Level Up Dance Studio.*

I hereby grant permission to Level Up Dance Studio and its employees to take and use photographs and videos of me for promotional purposes without compensation. I further acknowledge that Level Up Dance Studio has security cameras installed throughout the facility and in all rooms other than the restrooms, and that footage may be stored for an indefinite period for various reasons, including but not limited to security, insurance, or quality control purposes.
